  • mailed if you choose to pay by check or money order, select Mail-In Application, otherwise,
  • you may complete an on-line application and pay by credit card using the PayPal secure on-line payment system
       Step 1.  Complete and submit the On-Line Application and then
       Step 2.  Use the Secure On-Line Payment
